Depressive feelings are common in anorexia nervosa, particularly if the condition has existed for a long time. There was only one patient where indications of a serious depression went back to early childhood. Anthony recently described adolescent depression as evolving out of the development phases of childhood, rather than those of adolescence itself, although they may be overlapping. He described how well-defined clinical depression may develop in those who enter the adolescent stage already sensitized to depressive reaction. One of his patients, oversensitive to fear of rejection, separation, and loss of love, suffered a severe weight loss, from 123 lbs to 55 lbs, in association with a depression.

In the case of a 34-year-old Negro woman, severe economic and social problems combined with depression in producing a picture that superficially resembled anorexia nervosa. She was admitted to the medical service with anemia and cachexia, weighing only 65 lbs. This severe malnutrition had developed over a period of 5 years, after her marriage had broken up. She lived in the same household with her invalid mother and a psychotic sister. She had married 10 years earlier and her husband went overseas for 3 years of military service; he left her 2 years after his return. There was much tension between the patient and her sister.

There are widespread feminist organizations like the Feminists Anti-Censorship Taskforce (FACT) that strongly oppose those feminists who join organizations like Women Against Pornography (WAP). As we discussed in chapter 4, men have for millennia restrained women by stressing that they must not have sex without being in love while allowing themselves a more pleasure-centered approach to sexuality. What the feminists opposed to erotic pluralism are saying supports this ancient restriction on women. It is for this and other reasons that many feminists reject this traditional "feminine" constraint and argue for their right to pursue sexuality as they individually see fit.

The supports of inequality are not in erotica. Inequality is rooted in the limited power granted to women in our economic and political institutions. Nan Hunter, an American Civil Liberties Union attorney and a leader of the Feminist Anti-Censorship Taskforce (FACT), testified at the 1985 Meese Commission hearings: Pornography is not the cause of oppression in women nor is it even the primary channel in which that supremacy is reflected, validated and glorified. Other much more powerful, established and legitimate institutions contribute far more than does the pornography industry to the second-class status of women. Thus we and many other feminists believe that targeting pornography...
anti drug essay
The National Institute on Drug Abuse (NIDA) has been at the forefront of efforts to find effective techniques that will enhance drug treatment retention, participation, and outcome. The Substance Abuse Intervention Programs (SAIP) at Wright State University School of Medicine and the Polysubstance Rehabilitation Program (PRP) at the Department of Veterans Affairs Medical Center (DVAMC), Dayton, Ohio, are currently partners in a NIDA-funded demonstration research project. Called the Enhanced Treatment Through Induction and Case Management Project (Enhanced Treatment Project), its goal is the improvement of treatment retention, compliance, and outcome.
essay on teenagers drug abuse
Drug abuse is a response to a series of social and psychological disturbances, and long-term residential treatment, rather than case management, would appear to be the most appropriate approach. It would appear, however, that assertive case management holds better promise as an aftercare approach for drug-involved offenders following more intensive primary treatment. In any case, the practical limitations of the inability to require participation preclude a real test of the intervention. This points to the need to retain criminal justice clients in treatment. One consensual finding from treatment research is that the longer a client stays in treatment, the better the outcome in terms of decline in drug use and criminality

While WIP may be an effective means of motivating patients for the early phases of treatment, additional services can support longer-term retention, compliance, and successful outcome. Case management and patient advocacy encourage substance abusers to remain in treatment and realize treatment goals. Case management/advocacy was chosen as an enhancement technique for this project based on the belief that many patients quit drug abuse treatment and/or do not comply with treatment regimens because basic needs are often neglected during the course of treatment. In traditional treatment programs, an emphasis is placed on substance abuse as a primary condition whose severity warrants exclusive focus…

A program for sex partners of injection drug users (IDUs) achieved dramatic increases in numbers of clients after a health educator with roots in the community was assigned to supervise recruitment. Third, several community staff members took very personal interests in their work due to concerns over the ways in which drug abuse and related problems were affecting their neighborhoods. Some had lost family members or friends to AIDS and identified strongly with the effort to limit the epidemic. Some recovering staff members became skilled at using their empathy and personal reputations in the community to influence clients to enroll in treatment.

Perhaps the most enduring lesson learned from implementing the Enhanced Treatment Project was that it could be done. We demonstrated that an academic institution could work effectively with a large service provider to improve the care of drug abuse treatment patients. We found that it was possible to integrate two very different patient management philosophies in the same treatment unit. Finally, we were able to introduce a data collection capability into a service delivery program in a way that neither interfered with its mission nor taxed it in any way. In its third year of operation, the Enhanced Treatment Project continues to provide high quality service and collect comprehensive data about a heretofore little studied population of drug abusers.

Furthermore, justice requires that similar classes of persons be treated similarly. We should not discriminate inappropriately with regard to who is tested and what substances are tested. For instance, if drug testing were deemed appropriate for nurses whose impaired performance could put patients at risk, then fairness would dictate that physicians also be tested. The principle of justice also highlights the inconsistency of drug programs that do not test for alcohol, even though alcoholism is a major cause of work-related accidents. It is difficult to test for alcohol because it and its metabolites are rapidly cleared from the body.
